⚠️ You need to setup form access and include your department in the form name.
Creating a Form
- Go to Content > Forms
- Click + New button
- Enter a name for your form
- Click Create
Adding fields
- Open the Edit tab
- Drag field types from the left panel onto your form.
- Click a field to configure its settings in the right panel.
Field settings
Click a field to see its settings in the right panel:
- Label - What users see above the field.
- Description - Extra instructions below the field
- Required - Check this box if the field must be filled out
Circled numbers on the image are associated with the ordered list of field settings.
⚠️ Setting form access
While building your form, you need to restrict who can edit it and view submissions.
- Open your form
- Click the Properties tab
- Click Access in the left sidebar
- Select the access group
Naming your form
Please be sure to give your form a unique name. We recommend [department - form name].
